Position Overview

We are seeking a highly motivated, independent Wound Care Nurse (RN or LPN) to join our team. This role is ideal for a nurse who thrives with autonomy, takes ownership of their schedule, and is confident working independently while delivering exceptional patient-centered wound care.

The ideal candidate is organized, self-directed, and passionate about improving patient outcomes through evidence-based wound management.

Key Responsibilities

  • Provide comprehensive wound assessments and develop individualized care plans
  • Perform wound treatments including dressing changes, debridement assistance (within scope), and ongoing evaluation
  • Monitor healing progress and adjust treatment plans as needed
  • Document all care accurately and timely in EMR
  • Collaborate with providers and interdisciplinary teams
  • Educate patients and caregivers on wound prevention and care techniques
  • Maintain compliance with clinical and regulatory standards

Qualifications

  • Active and unrestricted RN or LPN license
  • Strong clinical assessment skills
  • Excellent time management and organizational abilities

  • Reliable transportation

Preferred (Not Required)

  • Wound care certification (WCC, WOCN, etc.)
  • Experience in home health, long-term care, or outpatient settings
